Mobile Health routine scheduled hubs in the communities offers these services: 1. Blood pressure checks for hypertension. 2. Blood sugar checks for diabetes. 3. Health education 4. Referrals to care and social services. 5. Scheduling free screening mammograms for uninsured and underinsured individuals. We will also collaborate with Roper St. Francis Health Services Center to offer HIV testing at select hubs.
